If we learn that we have collected information from a person under 18, we will promptly delete that information.

Vitalyze Wellness provides regenerative wellness therapy services, including but not limited to IV therapy, regenerative treatments, and related health and wellness services offered through our partner clinic network. Our Services include:

• Online scheduling and appointment management

• Health intake forms and consultations

• Virtual consultations and webinars via Zoom

• Educational content and wellness resources

• SMS/text message communications (see Section 4)

• Payment processing for services rendered

4. SMS / Text Messaging Terms

4.1 Description of SMS Use Cases

When you provide your mobile phone number and opt in to our text messaging program, you consent to receive SMS/MMS messages from Vitalyze Wellness. Our text messaging program is used for the following purposes:

• Appointment reminders, confirmations, and scheduling updates

• Treatment follow-up messages and aftercare instructions

• Service updates, cancellation notices, and operational notifications

• Responses to your inquiries and customer support communications

• Promotional messages, wellness tips, special offers, and educational content (only if you have opted in to marketing messages)

• Webinar and event reminders and follow-ups

• Payment reminders and billing notifications

• Review and feedback requests following your visit

4.2 Opt-Out Instructions

You may opt out of our SMS messaging program at any time. To stop receiving text messages from us:

• Reply STOP to any text message you receive from us

After you send STOP, you will receive a one-time confirmation message acknowledging your opt-out. You will no longer receive text messages from us unless you re-subscribe. Please note that opting out of SMS messages will not affect other communications you may receive from us, such as email.

If you wish to re-join our SMS program after opting out, you may do so by texting START to the number from which you previously received messages, or by re-submitting your phone number through our website opt-in forms.

4.3 Message and Data Rate Disclosure

Message and data rates may apply. Message frequency varies based on your interactions with Vitalyze Wellness and the nature of the services you use. For appointment-related messages, you may receive 2–5 messages per appointment cycle. For marketing messages (if opted in), frequency will not exceed 8 messages per month. Your mobile carrier’s standard messaging and data rates apply to all messages sent to and from Vitalyze Wellness. We are not responsible for any fees charged by your carrier.

4.4 Carrier Liability Disclaimer

Mobile carriers are not liable for delayed or undelivered messages. Vitalyze Wellness and the mobile carriers (including but not limited to AT&T, Verizon, T-Mobile, Sprint, and all other carriers) are not responsible for any delays, failures to deliver, or errors in the content of any SMS/MMS messages. Delivery of messages is subject to effective transmission by your mobile carrier and is outside our control. We make no guarantee that messages will be delivered on time or at all. Carriers are not responsible for the content of messages sent through our program.

T-Mobile is not liable for delayed or undelivered messages.

4.5 Supported Carriers

Our SMS program is supported by most major U.S. mobile carriers. However, not all carriers may support all message types (e.g., MMS). Carrier coverage and support are not guaranteed.

4.6 SMS Data Privacy

We do not sell, rent, or share your phone number or SMS opt-in data with third parties or affiliates for their marketing purposes. Your SMS consent information is used solely for the purposes outlined in these Terms and our Privacy Policy.
